20th Birthday Celebration at the 2026 Fall Forum

PCC Members are invited to an exclusive pre-meeting lunch to reconnect with old colleagues, build new connections and foster stronger relationships between PC members.  Note: This event is open only to PCC members and invited guests.

PCC President and CEO Ann Greiner and PCC Board Chair Sean Hogan welcome PCC members, the PCC Board of Directors, PCC's founders, past award winners and other guests to the 20th Birthday Celebration at the 2026 Fall Forum.

In this 40-minute session, participants engage in multiple rounds of small-group conversations guided by leading voices in the primary care space—including past winners of PCC awards. These discussions build meaningful connections, explore how the event’s theme applies to their work and identify what attendees personally want to gain from their experience.

In this panel conversation, early leaders of efforts to spread the Patient-Centered Medical Home reflect on how they were able to build a multi-stakeholder movement and the importance of engaging employers, payers and others to help spread PCMH. The panel will also explore how key lessons from that movement can inform and shape the PCC's and its members' efforts to improve the nation's health by strengthening primary care.

High-quality, whole person primary care delivers better health by addressing all aspects of a person's health--whether its physical, mental or emotional. In this session, attendees will break out into small group sessions to discuss how the PCC, our members and partner organizations can promote policies and industry reforms that advance whole-person primary care--leading to healthier, happier families across the nation. Attendees are also encouraged to discuss innovative approaches their own organizations have put in place to ensure patients get access to comprehensive care that promotes better health. 

In the wake of H.R. 1, states are facing increased pressure to cut costs--especially in state Medicaid and safety net programs. In this session, panelists will discuss strategies states should implement to protect families that rely on the primary care safety net and preserve (and potentially expand) essential primary care investments that can generate better health and lower costs.
