Advanced Primary Care Management services—launched by Medicare in 2024—show enormous promise in supporting primary care to deliver whole-person primary care. By creating a single, bundled payment that eliminates the need for time-tracking, the new program aims to make it easier for primary care to focus on helping patients manage their care more effectively.

While the program has enormous potential, some primary care practices have experienced challenges implementing the program—especially mandatory cost-sharing. In a new fact sheet, the Primary Care Collaborative and West Health provide insights into how policymakers can help remove one of the biggest hurdles to widespread adoption of APCM.

On September 22, the Primary Care Collaborative (PCC) will host a webinar exploring the role of nutrition in a primary care setting and how clinicians can effectively implement nutrition-based interventions to enhance the health of their patients.

Evidence has long shown that nutrition-based interventions help improve health outcomes. However, some policymakers question how well such interventions can work in under-resourced communities.

The PCC is excited to host a conversation with health leaders showcasing the many successful ways primary care teams are partnering with patients to support healthy eating and overall wellbeing. Panelists will also answer the many questions that clinicians have regarding nutrition-based interventions, from efficacy to implementation.

Primary care medics are designed to bolster the workforce by complementing physicians, nurse practitioners, and physician associates at primary care clinics.

The Primary Care Collaborative (PCC) and American College of Clinicial Pharmacy (ACCP) developed a resource guide to support the integration of Comprehensive Medication Management (CMM) into advanced primary care models, including the Patient-Centered Medical Home.

CMM is a patient-centered, team-based approach that ensures medications are used appropriately, effectively and safely to optimize health outcomes and reduce costs.

This data brief presents findings from the 2025 Commonwealth Fund International Health Policy Survey of Primary Care Physicians to examine care coordination in 10 countries: Australia, Canada, France, Germany, Netherlands, New Zealand, Sweden, Switzerland, the United Kingdom, and the United States. We report on the types of clinicians coordinating care, the timeliness of care delivery, the information being exchanged between providers, and reported barriers to coordination. We also identify key lessons for how countries can improve care coordination.

The Primary Care Collaborative (PCC) hosted “Primary Care Wins in the 2027 MPFS,” an online webinar devoted to the proposed 2027 Medicare Physician Fee Schedule (MPFS). PCC President and CEO Ann Greiner, M.C.P., talked through the rule’s primary care provisions with Jake Quinton, M.D., chief medical officer for Medicare Parts A and B at the Centers for Medicare & Medicaid Services (CMS). A second panel, also moderated by Greiner, brought in Aisha Pittman, M.P.H., senior vice president of government affairs at the National Association of ACOs (NAACOS); Stephanie Quinn, senior vice president of external affairs and practice experience at the American Academy of Family Physicians (AAFP); and Nima Sheth, M.D., M.P.H., vice president of health justice at the National Partnership for Women & Families.

A new qualitative study published in The Annals of Family Medicine spotlights how day-to-day practice differs between U.S. primary care physicians working for health systems and those in independent settings.
