In January 2025, Medicare began covering Advanced Primary Care Management (APCM) services. According to the Center for Medicare and Medicaid Services (CMS), APCM services and related billing codes were designed to “better describe advanced primary care management services broadly, to provide more stability in payment and coding for practitioners.”

In its 2026 Evidence Report, the Primary Care Collaborative dives into the current state of APCM adoption; what factors are driving adoption; what challenges early adopters have faced; and how the primary care community perceives CMS’s latest effort to address chronic conditions.
